Dental insurance is great!  We will gladly assist by filing those complicated insurance claim forms for you and even accept assignment of insurance benefits so that your “out of pocket” can be minimized.  We will work diligently to obtain the maximum reimbursement possible; however, you must be aware that  . . .

We cannot bill your insurance unless you provide all insurance information.  Should you have any change in insurance information, it is your responsibility to inform the practice before your next appointment or service is rendered.

You should remember your insurance policy is a contract between you and your insurance company and we are not a party to that contract.  What that means is Dr. Hull and his out of network practice works for you and expects payment from you; furthermore, you are ultimately responsible for all fees for services rendered.

 

 

Concerning Insurance Co-payments

You will need to pay the patient portion and any deductibles on the date services are rendered with cash, check or credit card.  We will make every effort to obtain payment from your insurance company; however, should the full amount be denied or not fully satisfied, the balance of the account becomes your responsibility.

Please be aware, insurance companies often process claims and may consider your services cosmetic in nature or not medically necessary or fees charged may fall outside their determination of “customary and reasonable fees.”  Regardless of the outcome of your insurance company’s determination of benefits payable, you are required to pay the full amount charged by Dr. Hull’s practice.  The practice does not write off these fees.

If Your Dental Insurance Will Not Pay Dr. Hull’s Practice

Some insurance companies will not pay out of network providers.  Should we discover your insurance company will not pay Dr. Hull’s practice directly, you will need to pay for services on the date they are rendered and we will be happy to assist in the filing of your claim as usual.

 

 

Unpaid Balances

You will need to satisfy all account balances within 30 days of billing date.  After 30 days, there will be a monthly interest charge added to your balance.  Should your balance remain unsatisfied for 90 days, you may be turned over to collections, where your balance could double due to fees charged by the collection agency.  Once your account goes to collections, we are unable to reverse the standing of your account or accept payment directly, additionally; this will likely cause adverse consequences to your credit.
